Located at 1311 Durham Avenue in South Plainfield, NJ, AristaCare Cedar Oaks (formerly Cedar Oaks Care Center) is the newest of AristaCare Health Service’s acquisitions as of March 1, 2007. Our plan is to transform AristaCare at Cedar Oaks into a shining example of 21st century healthcare. Not only will we provide extraordinary levels of care, but we will do so in an ideal healthcare environment—a place our patients and residents will be proud to call home, and our guests will be pleased to visit.

Our unique acute level rehabilitation program is what distinguishes AristaCare at Cedar Oaks from all subacute rehab facilities. At AristaCare at Cedar Oaks, patients receive customized intensive, acute level therapy, seven days a week. This aggressive approach ensures better outcomes, and a faster recovery. Some of our unique therapy programs will include: Sports Medicine and Functional Rehabilitation Program, Aquatic Therapy, and Post Acute, Post-Op, and Subacute Rehabilitation and Care.

AristaCare at Cedar Oaks also offers a Comprehensive Stroke Recovery Rehabilitation Program - this unique and specialized stroke rehabilitation program thoroughly encompasses and addresses the many facets of recovery, including rehab, psychosocial, and clinical progression. Our goal is to aggressively rehabilitate the stroke patient’s complete functioning by utilizing the highest forms of technology, rehab equipment, sports medicine, targeted therapy programs, Aquatic Therapy, as well as a comprehensive array of educational and support programs. Our “Lifestyle Recovery Program” assesses each stroke patient’s pre-stroke living habits, and an individualized care plan and program is set up to empower stroke patients with the tools, knowledge, rehabilitation, and practice necessary to adapt to their new limitations, yet resume as many previous activities as possible. Family education and support is included and crucial for the most complete recovery of stroke patients.

AristaCare at Cedar Oaks is also proud to provide Specialized Cultural Programs for Indian and Jewish populations…both involve individualized care programs. For our Indian patients, we provide specialized Indian physicians and nursing staff, food, religious services, activities, decor, TV, atmosphere, and customs that are tailored to the Indian culture, enabling patients to maintain a connection with their preferred lifestyle. Being in this familiar environment enhances our Indian patients’ social lives and hastens their health and rehabilitation. For our Jewish patients we offer L’Chaim Lifestyles – a Jewish cultural program which provides an appropriate setting through the offering of specialized cultural activities, traditional cuisine (certified Kosher and Glatt Kosher available), Jewish religious services, Sabbath services, themed music, and suitable reading materials. Working in conjunction with the Chaplaincy Committee of the Jewish Federation of Central Jersey allows us to continually enhance, maintain, and improve the quality of Jewish life for our patients at AristaCare at Cedar Oaks, and throughout the community through individual visitations, group programs, holiday celebrations, Shabbat services and Judaic entertainment and experiences.

Our facility encourages family members to be actively involved with the progress and care of their loved ones. Visits from loved ones play such a vital role in the complete care of family members. We realize that many people do not have access to transportation, or are challenged when distance is a factor, which is why we offer complimentary family transportation to all of our patients’ families.
